How to Use a Tourniquet — The Right Way

Uncontrolled hemorrhage is the number one cause of preventable death in trauma. An arterial bleed can kill a healthy adult in less than three minutes.

If you are hunting, hiking, or off-roading, you are likely much further than three minutes away from definitive medical care. In these scenarios, a tourniquet is not just a piece of gear—it is the only thing standing between you and a fatal outcome.

But simply carrying a tourniquet isn't enough. As an ER physician, I have seen tourniquets fail. And almost every time, they fail because of human error. People hesitate. They place it too low. They don't tighten it enough. Or worse, they try to improvise with a belt and a stick when they should have carried the real thing.

Field Reality Check

The biggest myth in trauma medicine is that using a tourniquet means the patient will lose their limb. This is a lethal misconception left over from the Civil War. Modern data from the Joint Theater Trauma Registry proves that commercial tourniquets can be left in place for up to two hours with little to no risk of permanent nerve or tissue damage. Life over limb—always.

When to Apply a Tourniquet

According to Tactical Combat Casualty Care (TCCC) guidelines, a tourniquet should be your first treatment option when faced with life-threatening external bleeding from an extremity (arm or leg). Do not waste time with direct pressure or elevation if you see:

  • Bright red, pulsing, or spurting blood.
  • Blood pooling rapidly on the ground.
  • Clothing that is completely soaked through with blood.
  • Any traumatic amputation (even a partial one).

How to Apply It: High and Tight vs. 2-3 Inches

There are two accepted methods for tourniquet placement, depending on the situation.

Method 1: "High and Tight" (Care Under Fire / Immediate Action)

If you are in a chaotic environment, you cannot clearly see the wound, or the patient is wearing heavy clothing, do not waste time exposing the injury. Place the tourniquet as high up on the limb as possible (high on the thigh, near the groin or high on the upper arm, near the armpit) and tighten it down over the clothing until the bleeding stops.

Method 2: "2-3 Inches Above" (Tactical Field Care)

If you have a safe environment and can clearly visualize the exact source of the bleeding, expose the skin and place the tourniquet directly on the skin, 2 to 3 inches above the wound (closer to the heart). Never place a tourniquet directly over a joint (knee or elbow) or over a bulky item in a pocket.

Step Action
1. Route & Pull Route the band around the limb. Pull the strap as tight as humanly possible before you even touch the windlass rod. If you leave slack in the strap, the windlass will bottom out before it stops the bleeding.
2. Twist Twist the windlass rod until the bright red arterial bleeding completely stops. This will take immense pressure.
3. Secure Lock the windlass rod into the clip. Route the remaining strap over the rod and secure it. 
4. Time Note the time of application. This is critical information for the surgeons who will eventually receive the patient.

The 3 Most Lethal Mistakes

  1. Not tightening it enough because it hurts. A properly applied tourniquet is excruciatingly painful. It acts as a hydraulic press crushing the artery against the bone. If the patient is screaming about the tourniquet, it means they are alive to feel it. Do not loosen it.
  2. Leaving slack in the strap. If you do not pull the initial strap as tight as possible before twisting the rod, the tourniquet will fail. You cannot out-twist a loose strap.
  3. Using a belt or shoelace. Improvised tourniquets have a massive failure rate. Belts cannot be pulled tight enough, and thin cords (like shoelaces or paracord) will slice through the tissue, causing internal bleeding and massive tissue damage without stopping the arterial flow. Carry a real, commercial tourniquet.

How long can a tourniquet stay on before losing a limb?

Medical data shows that a commercial tourniquet can safely remain in place for up to two hours with minimal risk of permanent ischemic injury. Muscle damage begins to occur after the two-hour mark, but the risk of limb loss from delayed treatment of severe bleeding is much higher than the risk from proper tourniquet use.

Should I loosen a tourniquet to let blood flow back in?

Absolutely not. Never loosen a tourniquet once it has been applied to stop an arterial bleed. Any blood you let back into the limb will flow straight out of the wound. A patient suffering from severe hemorrhage needs every drop of blood they have left. Only a medical professional in a definitive care setting should remove a tourniquet.

Can I use a belt as a tourniquet?

No. Studies have proven that standard leather belts are ineffective at stopping arterial bleeding. They cannot be pulled tight enough, and they lack a windlass mechanism to apply the necessary crushing force. In an emergency, an improvised tourniquet is better than nothing, but it has a high failure rate. You should always carry a commercial, TCCC-recommended tourniquet.
